[menscouncil] Feedback survey for men's dinner

Hi,

I've created a brief survey that we can send to all attendees after the
dinner.

The goal is to find out what they liked best about it and how likely they
are to attend again. I also ask two more questions about the dinner and
then a number of questions about their involvement or interest in other
men's activities.
