[menscouncil] Feedback survey for men's dinner

Hey Bob
I like the form, thanks for putting it together.
I wondered about an alternative format such as having a specific question
about each thing, and allowing people to speak for or against each
eg
comments about the theme and sources
comments about the program features (videos, panel, discussion questions)
comments about the food
comments about drinks
comments about setup

But i recognize that the current form already allows people to speak of
those things if they have any thoughts
