[menscouncil] Feedback survey for men's dinner

I like Pat's idea about allowing for more specific comments.

I will add something at the end like, "Would you like to comment
specifically on the different aspects of the evening?"

Those that answer YES can get more specific without requiring each
respondent to answer five more questions.

I will edit and email it out later today.
